CANCELLATION POLICY
I understand that if I cancel within 24 hours of my scheduled appointment, I am subject to pay 50% of the scheduled service. If I do not show up to my scheduled appointment without any notice, I am subject to pay 100% of the scheduled service. I allow “COVE SKIN STUDIO” to charge my card on file if I violate these terms. If I am under the age of 18, a parent or guardian must sign this form on my behalf. This agreement will remain in effect for the service and for all future services conducted by “COVE SKIN STUDIO” from the date of the signed form.
